By purchasing these products, I agree to the following terms:

• Products acquired through this medical-channel distributor may be used only in circumstances where

breastfeeding is not possible.

• The products must not be displayed and must always be stored in a closed cabinet.

• Under no circumstances shall the medical-channel distributor price be passed on to consumers, in accordance

with Section 6(c) of Executive Order No. 51.

• The HCP's acceptance of this disclaimer will be recorded and tracked by the Carbon team.
